NEW EDCHOICE SCHOLARSHIP APPLICANT
STEP 1 – APPLY
All documents must be returned to St. John’s Jesuit High clicking the upload button below. If you have more than one student attending St. John’s Jesuit, you will need to submit forms separately for each. Application questions may be directed to Mary Ziemke at mziemke@sjjtitans.org.
- Complete the EdChoice Request Form 2025-2026. All forms must be signed, no electronic signatures will be accepted.
- Copy of the student’s Birth Certificate.
- Proof of Address – Full page of a current utility bill in your name less than 3 months old. It must include both a billing and service address.

- Custody Documents if Applicable: If biological parents are not living together and both parents are on the child’s birth certificate, either the mother must be the primary parent on the application or custody documents must be provided showing shared or joint custody.
- Complete Check Signing Consent Form – requested by St. John’s Jesuit authorizing the school to endorse and negotiate checks issued by the EdChoice program. One per student.

Documents will be reviewed in the order they are received. You will be notified if corrections are needed or if further information is needed.
STEP 2 – INCOME VERIFICATION
Traditional EdChoice Scholarship – Complete only if Low-Income Qualified
Families who qualify for low-income status do not have to pay any tuition that is not covered by the scholarship. You will be responsible for all fees. For the family to be considered for the low-income status, the family must submit their household income annually to the Department of Education and Workforce.
Expansion EdChoice Scholarship – Must be completed for all new applicants

- Use your Adjusted Gross Income (AGI) from your 1040 tax form. A 2023 1040 tax form is acceptable until April 15, 2025; after April 15, 2025 a 2024 1040 tax form is required.
- Dependents under the age of 18 may be entered into the Income Verification System as members of your household. If you have children up to 22 years old who are full time students, or disabled dependent adults living with you, you may list these individuals’ name and date of birth in the Comment Section next to parent name. They also must be claimed on your 1040.
- Families that wish to only apply for the minimum scholarship ($616.60 for grades K-8 or $840.80 for grades 9-12), but do not wish to submit income information can now complete Minimum Scholarship Award Affidavit, have it notarized and submit it via the income verification portal. This affidavit may only be used in lieu of income information for applicants that are applying for the minimum scholarship amount.
