EdChoice Renewal Applicant
Step 1 – Apply
All documents must be returned to St. John’s Jesuit by clicking the upload button below. Families with more than one student attending St. John’s Jesuit, will need to submit forms separately for each. Application questions may be directed to Kim Hall at khall@sjjtitans.org.
1. Complete the EdChoice Renewal Form 2025-2026. All forms must be signed, no electronic signatures will be accepted.
2. Proof of Address – Full page of a current utility bill in your name less than 3 months old. It must include both a billing and service address.

3. Complete the Consent Check Signing Form – requested by St. John’s Jesuit authorizing the school to endorse and negotiate checks issued by the EdChoice program.
NOTE: Please have all documents complete and ready to upload before clicking the Upload Documents button.
Documents will be reviewed in the order they are received. You will be notified if corrections or further information are needed.
STEP 2 – INCOME VERIFICATION
Traditional EdChoice Scholarship – complete only if low-income qualified.
Families who qualify for low-income status do not have to pay any tuition that is not covered by the scholarship. For the family to be considered for the low-income status, the household income must be submitted annually to the Department of Education and Workforce. Please note that every family is responsible for paying the non-tuition fees. For more information about fees, contact Director of Financial Aid Phil Anthony at panthony@sjjtitans.org.
Expansion EdChoice Scholarship for Renewals (Optional) – Unless a family has a decrease in adjusted gross income or an increase in your dependents, there is no need to complete the income verification process.

• Use your Adjusted Gross Income (AGI) from your 1040 tax form. A 2023 1040 tax form is acceptable until April 15, 2025; after April 15, 2025 a 2024 1040 tax form is required.
• Dependents under the age of 18 may be entered into the Income Verification System as members of your household. If you have children up to 22 years old who are full time students, or disabled dependent adults living with you, you may list these individuals’ name and date of birth in the Comment Section next to parent name. They also must be claimed on your 1040.
